Diabetes is among the leading causes of death, physical impairment, and preventable hospitalization in most communities. The Healthy People 2020 goal is to reduce the disease and economic burden of diabetes mellitus (DM) and improve the quality of life for all persons who have, or are at risk for, DM.  At the community level there are opportunities to pursue this goal through collaborative efforts for diabetes prevention, detection, and patient-centered treatment.  The resources below provide insights, ideas, objectives, and promising practices for community-based prevention and control of diabetes. These resources can be adapted to support collaborative community efforts to prevent and control diabetes.  Note: Links to cancer resources are also provided because many cancer prevention initiatives include tobacco, nutrition, physical activity, and screening that promote prevention of both cancer and diabetes.)
