  • We realize that not every staff member will be able to attend every call, which is we archive the didactic training information on the platform. You can access this and more from the http://chcleadership.com/pcmh-office-hours-content/
  • Also, you can email us or use the Request Support form on the platform to submit questions! We do our best to promptly respond directly to the center that asked the question, and we also share any non-proprietary learnings with the whole group.
  • These questions can be strategic in nature – considerations about 2014 vs 2017 and timing for application – or very specific to a certain criteria or element. We welcome it all and it actually helps us to know what you are working on and how we can tailor our supports.

 QUESTION: What is the CHS PCMH Knowledge Base?  Where is it on the portal? How do I use it?

The CHS PCMH Knowledge Base is a searchable database of documentation examples, frequently asked questions, tools and resources for primary care organizations pursuing NCQA PCMH recognition. You will find the CHS PCMH Knowledge Base under PCMH Resources on the CHCLeadership portal. You can search the CHS PCMH Knowledge Base by: Category (e.g. Documentation Examples, FAQs), PCMH Element (e.g. 1A, 4B), and Keyword http://chcleadership.com/the-pcmh-kb/

QUESTION: Do you have examples or guidance on what clinics have done in the past to meet Element E: Implement Evidence-Based Decision Support?  

For PCMH 2014 – 3E Factors 1–6: NCQA reviews:

  • The conditions that the practice identified for each factor.
    1. A mental health or substance use disorder. (CRITICAL FACTOR)
    2. A chronic medical condition.
    3. An acute condition.
    4. A condition related to unhealthy behaviors.
    5. Well child or adult care.
    6. Overuse/appropriateness issues
  • AND The source of guidelines used by the practice, for each condition.
  • AND At least one example demonstrating guideline implementation at the point of care for EACH factor (1-6) identified.

In our experience, we’ve seen clinics create a chart that Identifies the conditions that the practice identified and the source of the guideline they use at the point of care.

In addition to the chart identifying conditions and guidelines, you must also include examples that demonstrate implementation of guidelines.  For this, we have seen screen shots of the point of care reminders in the electronic record for EACH factor selected.
